What problem in healthcare are you aiming to solve?
• Women affected by breast cancer lack clear, continuous, and structured support throughout their journey.
• The breast cancer journey unfolds in three phases: diagnosis → treatments → post‑treatments.
• At each stage, the same issue emerges: information is everywhere and nowhere at the same time.
The journey is fragmented, and patients are overwhelmed: sorting through information, managing multiple administrative steps, and dealing with the negative impact on their professional and personal lives.
Consequences:
Difficulties managing daily life, feelings of abandonment, isolation, mental overload, and a severely compromised quality of life.
What solution are you proposing to address this problem?
“The application combines rigor and humanity: a space where information provides structure, words bring comfort, and each person becomes an active participant in their own journey.” — Fabian Echeverria
CareTogether is built on two inseparable pillars:
• A virtual guide that centralizes reliable, contextualized, and prioritized information based on each person’s profile and stage of the journey.
• A community of women affected by breast cancer who share experiences and useful information in a structured, moderated, and secure space.
This combination transforms an often chaotic individual experience into support that is useful, clear, and deeply human—without blurring the line between medical expertise and the experiential knowledge of patients.
How did H4 support you?
• Access to a network of experts, along with guidance and orientation.
• Support and visibility through the Défi Source program.
• Support in the search for funding.
In progress.
